Community Pharmacies Survey
This survey is for anyone who uses a community pharmacy in Derby City. This could be using Pharmacy First, collecting prescriptions or accessing any other services and products.
Healthwatch Derby would like to gather these experiences and share with providers, commissioners, partner organisations and other health and social care services to improve understanding, learning and development.
All information is given anonymously. Please give as much detail as you can to answer the questions.
